Mail-in rebate question?

I’ve just ordered an EVGA Geforce GT240 on amazon and it was said to be after MIR and this is my first time doing an Mail-in rebate so I don’t know how it works the rebate website is here: http://evga.4myrebate.com/customer/default.aspx and I registered for the rebate and the form required me to send it with copy of receipt/invoice or packing list,original serial number barcode label and Original UPC barcode label. Now my question is which receipt are they talking about,what is the original serial number barcode label? or the original UPC barcode label? also are these item suppose to be in the package when it delivered? the estimated arrival date of the package is sep 10-22 and the deadline is 30 days, so is it possible my rebate will be expired before it processed? Thank you for reading this and sorry for the long question but I need to understand this thoroughly

    The receipt is a printed piece of paper showing what you bought and how much you paid. If you had bought the item in a store, it would be from the cash register. The UPC barcode is the one on the box as you would see it in a store. The serial number barcode, you’ll probably figure that out when you see it.
